If you often feel burning in the eye, redness, eye fatigue, foreign body sensation, blurred vision ... you are probably suffering from dry eye, eye surface disease, or a condition in which there is a decrease in tear secretion or their quality.
Hormonal changes, various environmental factors, and lifestyle are the most common causes of dry eye. Why is a tear film important? It keeps the surface of the eye moist, supplies the cornea with oxygen and nutrients, and forms a protective barrier against microorganisms.
‘The mechanism of onset of dry eye symptoms is due to tear film instability and hyperosmolarity, which causes damage to the superficial epithelium by cascading activation of inflammatory cells on the surface of the eye, with consequent release of inflammatory mediators. Inflammatory mediators, on the other hand, destroy the cells that produce the components of tears and in that way additionally worsen the symptoms', explains a doctor, subspecialist for the anterior segment of the eye, noting that, according to the cause, we distinguish two basic types of dry eye: hyposecretory (reduced tear production) and hyperevaporative dry eye (increased tear evaporation).
In hyposecretory dry eye, the causes can be degenerative processes or inflammation of the lacrimal glands, various systemic diseases (Sy Sjogren), paresis of the facial nerve, and various drugs (antihistamines, oral contraceptives, antidepressants). Hypervaporative dry eye, on the other hand, is most often the result of degenerative or inflammatory disorders of the joint or eyelids.
'In treatment and therapy, proper diagnosis of the type of dry eye and individualization of the approach to each individual patient is required. There is a wide range of preservative-free artificial tears on the market today, which serve either as replacement therapy (as in the hyposecretory form) or to relieve symptoms (in the hyperevaporative form of dry eye). In severe forms of dry eye, and in order to reduce the inflammatory component of dry eye syndrome, short-term use of mild corticosteroids leads to relief of symptoms and reduction or cessation of inflammatory processes, 'explains the doctor.
